Our Flemington office is located on the ground floor of the Hunterdon Doctors Office Building in Suite G3, adjacent to Hunterdon Medical Center.
In order to prepare for your visit, established patients are required to arrive 15 minutes before your appointment and new patients are required to arrive 30 minutes before your appointment.
Upon registering with the receptionist, you will be requested to identify yourself and present your insurance ID cards with confirmation of your name, date of birth, address, telephone numbers and insurance information. Completion of a new Patient Information Form will be required annually in order to update and record any changes. If this is your first visit with us, you will also need to complete a Health History Form. Both of these forms may be completed ahead of time and may be downloaded by clicking on their links.
Referrals are to be presented before your visit with the physician. It is your responsibility to keep track of the number of visits allowed and the valid period of each referral (See HCA Payment Policy).
Co-pays as well as any fee for service‚ and non-covered service fees are due and payable at the time of service.
Referrals and co-pays are required from HMO subscribers for blood pressure checks, holter monitor hook ups, blood draw and finger stick pro-time checks.
Walk-in patients with requests other than medical records will be required to complete a request form that will be processed within 48 hours.
A fee of $20 will be charged for all returned checks.

Prescription renewals require 1 business day to process. Please call the office between the hours of 9AM and 4PM to request a prescription renewal. We are unable to renew prescriptions on weekends, therefore it is important that you anticipate your needs and call 2-3 days before your prescription is finished. Walk-in patients with requests for prescription renewals or copies of medical records will be required to complete a request form. We will process your request within 48 hours.
